Documentation Must Include
- Diagnosis or medical condition, and whether it is temporary or permanent.
- A description of functional limitations, specifically related to walking, standing, or weight-bearing capacity.
- Impact of major life activities, specifically if the condition substantially limits walking, standing, or navigating campus
- Use of any assistive devices (e.g., wheelchair, boot, crutches).
- Estimated duration of limitation (for temporary conditions).
- A clear connection between the condition and the need for transport assistance.
Duration of Service and Reevaluation
- Approval for Mobility Transport is granted on a quarterly basis.
- For students with temporary mobility limitations, continued access to services for subsequent quarters requires reevaluation and updated documentation from a licensed medical professional or DSP.
- Students may discontinue services at any time if they no longer require assistance.

Eligible Students
- Functional mobility impairments due to permanent conditions requiring assistive devices (such as wheelchairs or crutches) or
- Temporary injuries causing severe pain, instability, or non-weight-bearing restrictions.
- Conditions that impact gait, balance, or endurance and prevent safe ambulation
- Sight Impairment
Ineligible Students
- If the condition does not directly impair mobility.
- This includes students with non-mobility-related disabilities, upper-body injuries that don’t affect walking, and those seeking transport for convenience, fatigue, or environmental reasons.
- Walking without assistive devices and without significant pain or impairment typically disqualifies a student from receiving services.
- Requests must be rooted in clear, mobility-related needs supported by medical documentation.
